Essential duties and responsibilities include the following.
• Assist with maintaining accurate records on all referral sources, status of existing or pending referrals, admissions, discharges, and hospitalized patients as needed.
• Establish and maintain positive relationships with referral sources.
o Ensure that a primary liaison is maintained between the Company and referral sources.
o Involve other community agencies in the referral process as needed.
o Compile patient information from referral source(s) including specific patient demographics, insurance and/or payor sources in order to ensure quality of communications, information, and care for patients.
o Communicate information effectively to physicians, supervisors, and others in a timely manner.
o Assist with ensuring patients meet the face-to-face encounter regulations.
o Provide skilled nursing care to patients in accordance with physicians' orders and in compliance with company policies/procedures, state and federal regulations if requested.
o May participate in on-call rotation.
o Request additional information via telephone for referrals that are incomplete and document requests in the clinical record.
• Complete clinical record review/audits.
o Maintain proficiency with computer system.
o Comply with state and federal regulations.
o Knowledgeable of current Company policies and procedures and state and federal regulations.
o Maintain confidentiality of records, correspondence, et.
o Attend meetings and in-services as requested.
o Abide by all policies and procedures set forth by the Company.
o Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations.
o Manage and document phone calls from physicians, clinicians, patients, referral sources and communicate patient updates/new orders to clinicians.
o Follow up on labs and other clinical diagnostic tests, physician contact, and significant changes in the patient condition to ensure adequate physician notification, follow up and needed plan of care modifications and communicates such to clinicians.
o Review clinician visit notes to ensure timely, complete appropriate and accurate submission of documentation by visiting staff. Communicates findings/trends to Manager.
o Review, evaluate and supervise service delivery to ensure appropriateness of care and utilization of services, equipment, and supplies through activities such as medical record review and case conferences.
