Responsible for the provision of nursing care to patients in accordance with current BH Standards of Care. Administers and monitors patient response to therapeutic interventions. The RN demonstrates knowledge and application of the nursing process as it relates to patients and families; is accountable for professional nursing practice; collaborates with health team members, patients/families in the provision of care; delegates to and provides direct supervision to other health team members as appropriate in the implementation of the patient care plan; acts as an advocate for patients/families/significant others and practices within accepted ethical and legal parameters of nursing.
Clinicians practicing at this advanced level of care deliver exceptional care that is holistic rather than segmental care and that is collaborative in nature. The clinician recognizes the most important aspects of care and has an intuitive grasp on patient's needs. The nurse based on experience and understanding, recognizes the most important aspects of care. They often have an intuitive grasp of patient's /families/unit needs. They are patient champions and role model advocacy. They have solid communication and interpersonal skills. This description covers the essential functions of the position. Incumbent is expected to perform similar and related duties as assigned. The Manager/Director is responsible for ensuring that the RN consistently meets the necessary behaviors. They are seen as as the go to person. Demonstrate advanced knowledge in specialty area and have a broad range of operational and clinical issues.
1) Clinical Decision Making -
Provides comprehensive care to patients. Researches and incorporates latest health care and evidence based practices into practice.
Bases clinical decisions on assessment data that integrates past experiences and anticipates future needs.
Anticipates early and subtle signs and symptoms reflective of a changing condition and revises plan of care.
Precepts and develops practice specific competencies and measurement tools.
Demonstrates advanced knowledge in the care of patients and/or group of patients. Ensures consistency of care across the practice(s).
2) Clinical Practice Innovation -
Develops and incorporates creative and evidence based interventions into clinical practice.
Works across the organization to evaluate outcomes to improve performance, quality of care and patient health and satisfaction.
3) Nurse Patient Family Relationship -
Addresses complex patient/family clinical, social, behavioral and cultural needs and system issues when coordinating care across the continuum. Independently connects to needed resources.
4) Patient Family Education and Health Care Transition Management -
Provides patient, family or community education as requested or needed, assessing comprehension levels and customizing the education to the individual or group.
Develops methods, materials and programs to support team members working to transition patient to another provider or another level of service e.g. from inpatient to outpatient.
Provides patient centered care management.
5) Health Care Team Relationships -
Collaborates with the multidisciplinary team to develop service based care practices based on quality measures standards and evidence.
Develops staff and community education as requested or needed, assessing comprehension levels and formatting the education to the individual or group ability to understand the information.
6) Communication -
Communicates and coordinates with all entities involved in the care of a patient to promote and maximize care coordination and patient satisfaction.
Manages difficult conversations and resolves issues or recommends alternative solutions. Successfully deals with challenging interactions.
7) Leadership -
Represents the practice throughout the health care system. Conducts clinical in-services. Seeks out educational opportunities for the development of staff.
Participates in the performance evaluation of team members and in the selection, orientation, development and training as appropriate.
Assigns and oversees duties as appropriate. Initiates change based on emerging trends and data.
8) Advancement to Level III requires completed Outpatient PNRP worksheet, Clinical Narrative, two positive peer reviews, unit leader recommendation and a minimum of 5 points in the advancement components of professional activities for professional development.Required Work Experience:
1) The registered nurse must have four (4) or more years clinical experience as an RN with one of those years in Ambulatory settingPreferred Work Experience:
