Position Title: Coordinator, Population Health

Essential Functions: Under the direction of the administrative director, the Population Health Coordinator and Clinical Analyst supports collaboration, communication and care coordination with PHO physicians, community physicians and hospitals, patients, managed care companies and other members of the healthcare team. Their role is to assist the healthcare team by monitoring, analyzing, and validating claims and clinical data and coordinating the continuum of care for patients by proactively managing and coordinating quality measures to offer complete preventive care for all patients identified by the clinical integration platform. The Population Health Coordinator and Clinical Analyst utilizes predictive modeling and risk adjustment tools to coordinate health services and provide support, encouragement and education to achieve the highest level of self-management within their responsibilities for each identified patient population. Another key component of this position is the ability to identify and utilize appropriate data sources and/or systems, obtain the data electronically via various reporting tools, and effectively present accurate data in a manner that is meaningful and able to translate into positive action by clinicians in the medical management of patients to avoid gaps in care, reduce duplication of services, and increase quality and clinical outcomes.

Educational Requirements / Qualifications:

  • Bachelor's Degree

Licensure/Certification/Registration:

  • Florida Registered Nurse License

Special Skills/Qualifications/Additional Training/Experience Required:

  • The Population Health Care Coordinator and Clinical Analyst is a highly technical and clinical position that interacts with community physicians and hospitals, managed care companies, departmental leaders, clinical staff, and the Executive Group to provide data analysis and care coordination for effective decision making.
  • The position combines strong statistical, analytical and clinical skills to help the PHO optimize the delivery of services, better manage costs and improve outcomes.
  • Strong interpersonal and customer service skills are required to communicate with patients, family members, physicians and other health care providers, as well as other colleagues. Knowledge of medical terminology.
  • Must possess strong problem solving abilities and analytical skills to ensure patient's needs are met.
  • Must understand the medical home model, patient-centered health and wellness, and an integrated approach to care and increase health provider knowledge, skill and confidence in fostering enhanced patient outcomes, collaboration, and function. This position must understand and educate physicians on collaborative care models.
  • Strong understanding risk sharing and utilize clinical data to improve quality and coordination of care.
  • Knowledge of Medicare Stars Ratings, Healthcare Effectiveness Data and Information Set (HEDIS) metrics, and other performance measures sets.
  • Must possess strong computer skills to input and retrieve clinical information, as well as patient registration, appointment information, and managed care.
  • Utilizes critical thinking and good judgment in performance of tasks to assist stakeholders in higher outcomes, lower costs, and lower inappropriate admissions and readmission rates.
  • Must possess strong organizational skills and the ability to multitask and to prioritize daily assignments to ensure smooth work flow.
  • Knowledge and understanding of risk adjustment models, including Hierarchical Condition Category Models for medical record documentation and coding practices, predictive and financial modeling
  • Must be able to read, write, speak and understand English.
