Description

The RWJBH Institute for Prevention and Recovery ALT Patient Navigator will coordinate and facilitate the day to day navigator role of the ED-ALT Demonstration Program. Navigators will establish a system-wide Pain Collaborative, develop and implement novel, evidence-based approaches to treat painful conditions frequently diagnosed in the SBMC ED. Other duties include but not limited to: Train SBMC ED clinical staff on the utilization of opioid-sparing order sets and other identified evidence-based approaches, develop multimodal pain programs for the treatment of renal colic or calculus, back pain, dental pain, fracture, and abdominal pain. Respond to automatic referrals from the Electronic Health, provide ED patients with a diagnosis of renal colic or calculus, back pain, dental pain, fracture, or abdominal pain with a patient education guidebook and one-on-one education regarding pain management and coping strategies. Provide referral or referrals to patients with a need for and interest in medical and non-medical services related to a diagnosis of renal colic or calculus, back pain, dental pain, fracture, or abdominal pain. Participate in webinars and in-person workshops to share information regarding evidence-based alternatives to opioids utilized for pain management with staff from other RWJBH facilities

Qualifications:

Bachelor s degree required in behavioral health profession
Certified Alcohol and Drug Counselor (CADC) preferred
ASAM Knowledge and experience with ASAM placement criteria preferred
Must possess the knowledge, skills and experience necessary to completely perform case management
Minimum three (3) years demonstrated experience working with substance use disorder clients
