Job Description
Responsible for ensuring compliance with federal, state, and local laws and regulations, policies and procedures, and standards of accrediting body.
 Obtains, interprets and evaluates all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness.
 Initiates, reviews and updates all required documentation.
 Accurately maintains all legally required records.
 Communicates clinical and technical information and monitoring to other members of healthcare team and patient when appropriate.
 Participates in patient evaluation for acceptance, product selection, device selection and supply management.
 Performs accurate computer order-entry, labeling and dispensing in accordance with policies and procedures and state board of pharmacy regulations.
 Accurately compounds and checks compounded sterile products.
 Directly supervises pharmacy support personnel in the appropriate compounding, packaging and labeling of compounded sterile products.
 Follows established procedures for drug and supply procurement.
 Complies with FDA mandates and self-imposed manufacturer restrictions on particular drugs, as applicable.
 Maintains appropriate inventory levels and ensures use of preferred products.
 Maintains procedures for appropriate security of all legend items, including controlled substances.
 Participates in on-call responsibilities.
 Participates in Performance Improvement activities, educational programs and departmental/branch meetings.
 Participates in the branch assessment process as requested by pharmacy manager or designee.
 Communicates appropriately with all members of the healthcare team.
 Assists in training and orientation of new employees as requested by pharmacy manager or designee.
 Complies with the Company's policy on the allowable clean room garb and dress code requirements.
 Performs other duties and responsibilities as assigned.

Required Qualifications
Bachelors or Doctor of Pharmacy Degree in Pharmacy from an accredited college or university.
Active New York Pharmacist license .

Preferred Qualifications
One year related experience in an IV alternate site environment or equivalent is preferred.

B.S. or doctorate of pharmacy degree is required.
