GPS, part of Genesis HealthCare, one of the largest providers of skilled nursing and
rehabilitation therapy, is a group practice of Physicians, Nurse Practitioners, Physician Assistants
and others who specialize in the holistic care of sub-acute, skilled nursing and long-term stay patients.

Join the GPS team as an Advanced Practice Wound Provider where you will render acute and chronic wound care
services to nursing center patients in collaboration with the other medical providers. You will utilize evidence
based practice of care and guidelines.
*Evaluate patient health status, diagnosis, interpret labs and diagnostics
*Develop and implement patient treatment plans in collaboration with interdisciplinary care planning process
*Conduct weekly wound rounds and document in GehriMed according to GPS policy
*Assess, manage and treat wounds, identify wound etiology and cause of delayed/non-healing wounds
*Address quality of life concerns related to wounds and actively develop and implement curative,
maintenance, or palliative wound care goals to meet the patient/family goals of care, incorporating
advanced care planning
*Complete Genesis wound orientation, including Review of Center Framework for skin Integrity and approved Genesis Formulary products
*Serve as a clinical expert in wound care for nursing staff and provides targeted clinical education

*Must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner *Graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing is required. Concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP) is preferred. *Wound Certification: CWS, WOCN, or appropriate wound certification, or actively pursuing wound certification with APN wound experience. *A minimum of two years' experience in wound care, working with the adult or geriatric population is preferred. *In states where practice allows; APRN with an unrestricted professional license and DEA, for each state of practice.
