The following job functions are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

  • Plan, design, implement and evaluate education programs/activities relevant to the learning needs of all participants, using age appropriate learning methods, and sets short and long-term educational goals and objectives.
  • Assist with the development of programs to meet organizational goals
  • Understanding the necessity of resource utilization and cost containment measures to ensure fiscal responsibility
  • Act as a client advocate coordinating and guiding care across the continuum
  • Utilize and manage available resources, including the expertise of other professionals, to meet educational needs of all participants.
  • Serve as group leader, facilitator, instructor, or member on program planning teams, research projects and committees as assigned.
  • Ensure Lakeland achieves and maintains health care accreditation and adheres to state and federal regulatory requirements. Ensures adherence to the corporate compliance plan.
  • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Evaluate/Track all assigned programs using appropriate quality improvement procedures and data adhering to the organization's published values.
  • Perform other duties as assigned.
  • Based on specific areas assigned is responsible for the following:

Diabetes Educator

  • Provide individualized diabetes self-management education/training to individuals and groups according to the scope of practice, standardized of practice, and standards of professional performance.
  • Incorporate the AADE (American Association of Diabetes Educators) guidelines for accredited diabetes programs.
  • Serve as a clinical resource and expert in diabetes care to provide consultation to healthcare providers and healthcare consumers (in-patient and out-patient, as assigned)
  • In-depth knowledge about current diabetes treatment management applied to both in-patient and out-patient settings
  • Collaborate with all care team members and community organizations to provide leadership to all staff in order to provide a direction and plan of care for diabetes patients
  • Provide diabetes technology support
  • Monitor and track outcomes, working to decrease certain manifestations from poor control of blood sugar levels

Knowledge, Skills and Abilities Required

License/Registration/Certification Required: Current Registered Dietitian Nutritionist (RDN)

License/Registration/Certification Preferred: CDE or BC-ADM, insulin pump certifications, must obtain within two years of employment

Advanced Education Required: Bachelors in a nutrition-related field

Advanced Education Preferred: Masters in a nutrition-related field

Minimum Experience Required: minimum of five years in patient education, care management, or similar experience with a minimum of 2 years in specialized diabetes care

Additional Experience Preferred: Ideal candidate will have experience in both the impatient and outpatient settings.

Abilities:

Outstanding verbal and written communication skills, including public speaking, required. Strong customer service orientation required. Ability to travel locally and to all Lakeland facilities.

Ability to work extended hours beyond normal work schedule to include, but not limited to, evenings, weekends, extended shifts and/or extra shifts, sometimes on short notice.

License/Registration/Certification: Current applicable licensure in the State of Michigan. Certification in the specific clinical area preferred.

Ensures adherence to the corporate compliance plan. • Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service. Evaluates/Tracks all assigned programs using appropriate quality improvement procedures and data adhering to the organization's published values. • Based on specific areas assigned is responsible for the following: Laboratory Educator A. Establish appropriate systems for oversight of orientation and annual competency records for laboratory associates to include management of technical analyte checklists by individual. The records system must meet laboratory accrediting standards and corporate accrediting standards as defined. B. Coordinate in-services for new equipment, procedures, and competency assessments for identified job duties and Regulatory requirements. Respiratory Educator A. Incorporate the AARC (American Association of Respiratory Care) clinical guidelines. B. Coordinate in-services for new equipment, procedures, and competency assessments for identified job duties and Regulatory requirements. Diabetes Educator A. Provide individualized diabetes self-management education/training to individuals and groups according to the scope of practice, standardized of practice, and standards of professional performance. B. Incorporate the AADE (American Association of Diabetes Educators) guidelines for accredited diabetes programs. C. In-depth knowledge about current diabetes treatment management. Community Educators A. Conduct programming and screenings based on CHNA (Community Health Needs Assessment). Requirements

• Environmental Conditions: Potential exposure to fumes, dust, gases, and odors. Frequent changes in temperature may occur. Possibility of exposure to communicable diseases.
