1. Collaborates in the assessment of patient/significant other in a timely, consistent and systematic manner.
1.1 performs patient assessments as indicated by the patients condition or as directed by the RN.
Notifies physician of admission and obtains admitting orders per departmental policy.
1.2 Gathers data to be used in assessments for each patient each shift or more frequently as directed by RN.

Collaborates in the development of an outcome oriented plan of care that reflects the patients actual or potential health problems
2.2 Reviews the plan of care for assigned patients, collaborating with the RN in revisions as necessary.
2.3 Involves the patient and/or significant others in decisions regarding care.
2.4 Prioritizes care delivery according to the patients needs and unit activities.
2.5 Demonstrates the ability to interpret information in terms understandable to the adolescent, adult and geriatric patient.
2.6 Assists in planning care based on knowledge of growth and development changes occurring across the patients life-span.
Implements the plan of care under the direction of the RN to achieve the identified outcomes for the patient according to policy and procedure.
3.1 Provides holistic nursing care to an assigned group of patients according to the prescribed nursing plan
of care under the direction of the RN.
3.2 Assists with and/or performs procedures.
3.3 Provides education based on addressed needs including the safe use of equipment used in treatment or health maintenance.
3.4 Demonstrates ability to provide care and information based upon physical, psychosocial, cultural, safety and other related factors for clients.
3.5 Demonstrates knowledge of medications and their correct administration based on the age of the patient and his/her clinical condition.
3.6 Follows the five medication rights and reduces the potential for medication errors.

4.1 Collaborates with the RN in evaluating patient/significant others responses to care and in determining

4.3 collaborates with the RN in evaluating the effectiveness of discharge plans for meeting patients needs for continuing care.
4.4 Collaborates with the RN in obtaining data on patient satisfaction.
Demonstrates effective and accurate verbal, written and electronic communications
5.1 Creates a positive climate for communications.
5.2 Maintains the confidentiality of patient, staff and hospital information.
5.3 Communication is timely, accurate, concise, complete and appropriate.
5.4 Demonstrates skilled use of the telephone, Meditech and intranet.
5.5 Utilizes constructive problem solving techniques to promote change and address issues, concerns and problems.
5.6 Completes documentation of care and other pertinent data according to organizational policies and procedures.
5.7 Communicates significant changes in patient condition and/or medical orders to the RN in a timely manner.
5.8 Maintains relevant knowledge of hospital forms. Completes patient care documentation forms and employee/patient incident reports in a timely and accurate manner.
5.9 Treats patients/family members and physicians/staff with respect, maintaining a positive influence and work environment at all times.
Participates in unit and organizational activities.
6.1 Participates in 75% of staff meetings.
6.2 Participates in Performance Improvement activities such as data collection, inservices meetings, etc.
6.3 Participates in a hospital committee, special project or service as requested.
Complies with organizational policies and procedures.
7.1 Maintains current certification in CPR.
7.2 Attends all mandatory inservices.
7.3 Complies with all organizational policies, procedures and protocols and has no documented violations.
7.1 Reports to work on time and as scheduled.
7.2 Adheres to dress code; appearance is neat and clean.
7.3 Accurately completes clock in transactions.
7.4 Adheres to OSHA standards regarding appropriate safety regulations and procedures.
7.5 Completes required annual competencies on a yearly basis.
7.6 Maintains hospital property and hospital environment in a safe, clean and intact condition.
Demonstrates continued development of knowledge and skills.
8.1 Uses current knowledge achieved through educational activities to enhance or improve clinical
practice or patient care.
8.2 Participates in unit development and decision making through such activities as attending staff meetings, serving on a committee or task force and/or special project.
Demonstrates the knowledge and skills necessary to provide care based upon the physical, psychosocial, educational, safety and other related criteria appropriate to the following age groups: Neonatal, Pediatric, Adolescent, Adult and Geriatric.

Because you're committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That's why you'll bring to your role the right set of qualifications:
Education: Graduate of an accredited school of nursing
Licensure: Licensed to practice as an LPN in the state of Oklahoma
Certifications: CPR through the American Heart Association. Must have completed IV Therapy Program with documented return demonstration for clinical competence.
Other: 1. Demonstrates professional conduct and development. 2. Able to learn and react quickly in the performance of multiple tasks. Competent to make certain independent decisions within the realm of licensure and seeks assistance as required. The LPN will function autonomously, but must realize that the registered nurse in responsible for the overall plan of care. The LPN follows the direction of the RN and the unit/hospital guidelines. Effective interpersonal skills are required for interfacing with patients, staff, physicians and visitors. 3. Must be able to engage in technical conversation involving discussion with physicians, nurses, patients, families/significant others and co-workers.
Preferred Experience: 1 year medical-surgical experience preferred.
Preferred Certifications: ACLS within one year of hire date PALS within one year of hire date
