Our experienced staff of Psychiatrists, Psychologists, Social Workers, Mental Health Specialists, Parent Partners and other therapists/counselors provide case management, mental health care and a range of support services in our outpatient clinics and 24/7 treatment facilities, and in community settings such as schools and homes. Our dedicated teams work collaboratively and in partnership with family members, school administrators and faculty, county social workers, case managers, probation officers and other community-based agencies.

JOB SUMMARY: This position is responsible for all aspects of medical record management for SVAC. Essential duties of this position are to create charts, assure that all aspects of the record are in keeping with agency policy and DMH requirements, management and control of forms, thinning charts, auditing, monitoring, tracking, and securing medical records, maintaining active and closed charts organization, assisting in acquiring necessary documents from charts when authorized, and assists in preparing discharge packets. Activities performed can be time sensitive with expectations of frequent reporting of progress along with thorough oversight for accuracy. Regular interaction with all staff members, physicians, and outside agencies is expected. Handling extensive personal and confidential information is an essential job function of this position.
MINIMUM QUALIFICATIONS
Education High school graduate or equivalent required. Associate degree in office management or other related field of study preferred.
Experience One year of general office experience required.
License or Certification Valid California Driver License. Current CPR Certification or ability to obtain within 30 days of employment. First Aid Certification of ability to obtain within 30 days of employment. Medical Terminology Certification preferred but not required.
Specialized Skills Ability to interact and communicate both verbally and in writing with seriously emotionally disturbed adolescents, other staff, physicians, SBHG consultants, and the public. Exercises good judgment in accordance with maintaining client/family confidentiality – recognizes and has the ability to maintain appropriate professional boundaries when communicating with clients/families, staff, and the general public. Demonstrated familiarity with computers, required. Efficient at Word, Power Point, Outlook, & Excel.
Physical Requirements Ability to sit most of the working day. Ability to bend and stretch to retrieve records from shelving six foot high.
POTENTIAL JOB HAZARDS Assaultive adolescents Blood and Body Fluid Contact (Category III) Potential for strains/sprains from bending and stretching, and minor lacerations from paper cuts.
SAFETY PRECAUTIONS REQUIRED Demonstrated knowledge of response to assaultive behavior. Demonstrated use of Standard Precautions. Demonstrated knowledge of safety procedures regarding bending and stretching and Material Safety Data Sheets particular to departmental hazards.
EMR DUTIES AND RESPONSIBILITIES when applicable 1. Scan into the Electronic Medical record all required forms submitted as part of the Private Health Information that establishes the permanent records for all clients. 2. Scan Semi-Annual and Annual packets 
3. Audit Medical Records portion of packets that are submitted for accuracy and completeness before scanning 4. Stamp and sign all scanned packets and other documents immediately upon scanning 5. Scan any required psychiatric documentation into the Electronic Medical Record as needed 6. Coordinate and communicate with the QA department and Center Coordinators to ensure that the Packet process is conducted in an appropriate and timely way 7. Coordinate with supervisors and clinicians to ensure identified and required corrections are completed in a timely, manner prior to scanning 8. Effectively use computer for coordinating all EMR related duties, including receiving and responding to all relevant emails 9. Document and log all request for deletions of scanned documents as needed 10. Provide training on the scanning process for backup staff as needed
DUTIES AND RESPONSIBILITIES 1. Monitors security of medical records, authorized use, and access control of medical records room 2. Supervise all access to Private Health Information by employees 3. Provides sign-in/sign-out sheets for charts 4. Visually audits all admissions and discharges within prescribed period for Assists in designated chart requirements 5. Manually creates chart on admission and assembles completed charts into folders with documents in correct order on discharge 6. preparing computerized daily census report for Public Folders 7. Keeps charts in order – active, thinned, and discharged – by medical record labels with SIMON number, client name, tabbed, and organized with Chart Order Checklist 8. May act as a backup and/or support person to Medical Records Technician as needed 9. May be responsible for other medical records-related clerical duties as reasonably assigned 10. Supports in placing documents in charts as needed and monitors appropriate chart filing by others with ongoing communication to fellow employees the team effort needed to maintain organization of the chart 11. Assists Medical Records Technician with entering client information into StarBase 12. Aids in logging incident reports onto StarBase 13. Helps with acquiring necessary documents from charts for I-Tracks and UORs 14. Assists in duplicating records when authorized (i.e. client attorney, CSW, subpoena, new placement, DSS) and supports in reporting al l information regarding copying and distribution of confidential material in the Release of Information Correspondence Log 15. Implements schedule to review all active charts organization and reorganize if necessary 16. Tracks and monitors charts to be thinned and removes old documents as needed 17. Communicates effectively with clinical and non-clinical staff regarding medical record issues 18. Coordinates duties and functions as a support and back-up person to Medical Records Technician 19. Interacts with clients/families, staff, and the general public within appropriate professional boundaries (i.e. ensures confidentiality, pleasant approach, maintains professional relationship) 20. Assists in pulling and preparing charts for internal and external audits 21. Participates in agency efforts to improve quality 22.
