Our experienced staff of Psychiatrists, Psychologists, Social Workers, Mental Health Specialists, Parent Partners and other therapists/counselors provide case management, mental health care and a range of support services in schools, homes or other community settings.  Our dedicated teams work collaboratively and in partnership with family members, school administrators and faculty, county social workers, case managers, probation officers and other agencies.


Central Star Psychiatric Health Facility (PHF) is a 16-bed facility that provides acute inpatient psychiatric services to adolescent clients ages 12 -17.



The Charge Nurse is the treatment team member who is responsible for performing all nursing duties and related nursing activities for clients.  The Charge Nurse is also responsible for the supervision of the Youth Counselor staff assigned to their respective shift.  





Graduation from an accredited school of registered nursing, required.

Bachelor's degree, preferred.



One (1) year experience with seriously emotionally disturbed adolescents, in an inpatient health care setting, preferred.

One (1) year supervisory experience preferred.


License or Certification

Valid California RN license.

Ability to pass facility Medication Test within 30 days of employment. (?)

Current CPR certification.

First Aid certification or ability to obtain within 30 days of employment.


Physical Requirements

Ability to physically perform containment, escort, and restraint procedures with assaultive adolescents.

Ability to physically assist in lifting and carrying assaultive adolescents weighing up to 200 lbs.

Ability to visually and audibly assess adolescent's behavior and needs.

Ability to stand for long periods of time preparing and administering medications.

  1. Work with physicians and social workers to secure informed consent for medication from parents, courts, or conservators.

  2. Take physician orders for medications, seclusions and restraint, denial of rights, medical procedures, lab work, x-rays, and ensure each order is implemented.

  3. Visually and audibly observe adolescents physical and psychiatric conditions and report changes to the Psychiatrist or other Medical Doctor.

  4. Perform documentation that records daily client outcomes. Administer medication as prescribed. Order and ensure on-going supply of individual medications for each client. Take vital signs as ordered.

  5. Initiate seclusion and restraint in an emergency. Monitor all seclusion and restraint activity including 15-minute checks.

  6. Audit Medical Records to ensure all orders have been implemented.

  7. Provide medication education and information on health practices to clients.

  8. Complete Nursing Assessments on new clients or when clients require such assessments.

  9. Compile Transfer or Aftercare Instructions for clients being discharged from facility.
