Provides skilled nursing interventions and focused assessments under the supervision of the RN in accordance with the plan of care.

1:1 Completes assessment for each patient as visit is made

1:2 Documents changes in patient condition as they occur, report changes to RN/Case Manager.

1:3 Perform skilled nursing care per qualifications based on the plan of care following established policies, procedures and protocols

1:4 Involves the patient/family in care decisions

1:5 Reinforces anticipated outcomes/goals

1:6 Follows scheduled visit frequency

Provides care and services in a continuous and coordinated manner.

2:1 Communicates to RN/Case Manager, Supervisor, or on call nurse immediately of any condition changes, new problems or complaints

2:2 Communicates schedule changes to RN/Case Manager, Intake Coordinator, or Nurse Manager as required

2:3 Gives verbal report on patient progress weekly to RN/ Case Manager

2:4 Participates in case conferences weekly

2:5 Updates in-home chart of any problems, instructions, or vital sign findings

2.6 Documents and/or updates medication record during each visit. Instructs patients and families on side effects for each medicine and documents any med change.

2.7 Notifies physician and case manager if medication problems are identified and/or when a patient's condition warrants.
Demonstrates the knowledge and skills necessary to provide the care or services.

3:1 Maintains working knowledge of home health policies, procedures and protocols

3:2 Completes orientation requirements

3:3 Attends staff meetings, in-services, and case conferences.

3:4 Maintains licensure and certification as necessary

3:5 Maintains annual job competencies

Complies with all policies and procedures related to the patients clinical record.

4:1 Ensures the confidentiality, security, and integrity of the patient
4:2 Complies with documentation time standards

4:3 Keeps chart updated and current

4:4 Completes verbal orders according to policy

Improves patient health outcomes by identifying and reducing the risk of infection in patients.

5:1 Performs techniques competently and in accordance with policies, procedures and protocols

5:2 Reports signs/symptoms of infections immediately to RN/Case Manager and/or physician as directed

5:3 Reports any communicable diseases immediately to supervisor

5:4 Follows infection control plan for home health

5:5 Complies with employee health policies

Performs other duties as assigned

Education: High School Graduate Accreditated LPN school graduate
Licensure: LPN licensure
Certifications: American Heart Association BLS Certification
