Job Summary

The LPN Extender provides practical nursing care within the scope of a LPN under the South Carolina Nurse Practice Act in the acute care setting. The LPN Extender functions as a collaborative member of the healthcare team in partnership with and under the supervision of a RN to provide practical nursing care for a specific group of patients. Assumes responsibility for data collection related to patient status, on-going monitoring, measuring and documentation of patient care. Provides patient/family instruction, as directed by the RN. Assists with activities of daily living, non-critical care transport and patient discharge.

Accountabilities

  • Performs basic bedside care and selected procedures under the direct supervision of the registered nurse to include, but not limited to: hourly rounding, basic blood glucose collection, coughing /deep breathing, incentive spectrometer, pulse ox and oxygen set up, dressing changes, recognition of life-threatening conditions with initial response and reporting, and emptying and measuring drainage equipment. Reports changes in patient condition to the Registered Nurse in a timely manner. - 10%
  • Accurately completes and documents patient vital signs including blood pressure, pulse, temperature, heart rate, and oxygenation via pulse ox or ETCO2. Reports abnormal values to the Registered Nurse. - 10%
  • Administers medications per policy and as delegated by the Registered Nurse (RN). - 15%
  • May perform individualized unit-based skills based on the type and needs of unit after training and competency validation such as: Foley catheter placement and removal, phlebotomy, EKG, IV insertion, maintenance and discontinuation, monitoring of telemetry rhythms or adjusting fetal monitoring. - 10%
  • Participates in patient rounding and, under supervision of the RN, actively engages in review of harm prevention measures which may include tasks such as: efficient turning and re-positioning, monitoring central line dressings for soiling or VAP (ventilator- associated pneumonia) Bundle. - 10%
  • Maintains and ensures accurate maintenance of patient's medical records and treats patient records as confidential. - 10%
  • Assists patients with needs related to personal hygiene including bathing, washing hair, brushing teeth and washing hands. Assists patients with needs related to nutrition and feeding including setting up meal trays and assisting with feeding.  Meets patients' needs related to elimination including assisting patient with getting to the bathroom, utilizing a bedside commode, bedpan or urinal.  Accurately monitors inputs and outputs.  Assists patients with needs related to ambulation including dangling, walking, walking with assistive device, and implementing fall prevention measures under the direction of the RN. - 30%
  • Functions as member of health care team and as a role model, mentor and leader to others in the same occupation. Participates in unit- and system-based performance improvement activities and training of new patient support team members as needed. - 5%

Supervisory/Management Responsibilities

  • This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

Completion of a LPN program recognized by the SC State Board of Nursing.

One year experience as a LPN.

Required Licensure/Registrations/Certifications

Licensed to practice as a LPN in South Carolina.

In Lieu Of The Above Minimum Requirements

In lieu of 1 year of LPN experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency program from another organization.

In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing.
