Description
The LPN Care Coordinator identifies all patients of Thompson's Family and Internal Medicine practices who are admitted to and discharged from local hospitals, Emergency Departments and Nursing Homes. Contacts specified patients to perform medication reconciliation (via phone or in person) within 48 hours of discharge, schedules follow-up appointments within five (5) days of discharge, collects all relevant inpatient or other documentation ensuring these are in place for morning huddles and identifies patients who could benefit from long term care coordination referring them to the RN Care Managers.

Required Job Specific Competencies:
- Provide on-site and telephone medication reconciliation to recently discharged patients
- Interact closely with clinical teams to identify gaps and improve clinical outcomes for patients with chronic conditions
- Support Thompson Practices quality programs, including PCMH and Diabetes Recognition program
Qualifications:
- Current New York State LPN license required
- CPR/BLS certification
- Proficient in the use of Electronic Medical Records (EMR); ability to create reports on own
- Competent in Microsoft Office products (Outlook, Word, Excel).
Education:
- Graduate of an approved school of Practical Nursing required
Experience:
- 3- 5 years of clinical nursing experience preferred
- Experience with the adult patient population preferred
- Community health nursing experience preferred
- Electronic Medical Record experience required

Position Pay Range: $23.00-31.00/hour
