Summary of Position:

This position provides patient-centered behavioral health services, in collaboration with the Patient Centered Medical Home (PCMH), to adult patients at Barton Community Health Center. The position requires assessment, conceptualization, treatment planning, self-direction, crisis intervention, using evidenced-based therapies within professional scope of practice, adherence to W&I Codes and maintaining professional and ethical behavior in accordance with the National Association of Social Workers, with little to no supervision. At least 60% of workday will be scheduled to provide billable psychotherapy services; the remaining 40% of workday will be directed at crisis intervention, providing social service interventions and coordinating collateral resources with patients, triaging and managing referrals and maintaining relationships with necessary community agencies.

Qualifications

Education:

  • Masters in Social Work from an accredited institute.

Experience:

  • At least 2 years of experience post licensure providing evidence based brief therapies and crisis intervention, with moderate to severe mental health disorders.
  • Ability to perform Motivational Interviewing, Solution Focused Therapy, Cognitive Behavioral Therapy and Dialectical Behavior Therapy, with little to no supervision.
  • Ability to assess suicidal/homicidal risk and other high risk situations (psychosis, drug/alcohol use/intoxication), create safety plans and adherence to W&I code, Section 5150 regulations, with little to no supervision.
  • Ability to diagnose using DSM-5 and create a treatment plan consistent with diagnosis.
  • Knowledge of community resources.
  • Ability to maintain electronic records necessary to support billing and documentation requirements.
  • Ability to perform position requirements with minimal supervision.
  • In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred.

Knowledge/Skills/Abilities:

EXAMPLES OF JOB DUTIES – Duties may include, but are not limited to, the following:

  • Knowledge of Epic EMR preferred.
  • Experience with group and individual therapy.
  • Working knowledge of psychotherapy billing and coding.
  • Prior experience with multidisciplinary teams and PCMH
  • Microsoft office and google doc's experience preferred.
  • Ability to effectively communicate in English, in compliance with patient safety standards. Bilingual abilities preferred.
  • Prior experience with addiction therapy preferred.

Certifications/Licensure:

  • Current licensure with the Board of Behavioral Sciences as an LCSW in the state of California, in good standing.
  • Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.
  • Masters in Social Work from a program accredited by the Council on Social Work Education.

Essential Functions

  1. Provides consistently exceptional care at all times.
  2. Provide direct behavioral health services to adult patients, including: diagnostic assessments, treatment planning, evidenced based therapies and crisis evaluation, with little to no supervision.
  3. Perform evidenced based brief therapies for 8-15 sessions, from intake to discharge.
  4. Thorough documentation of services, completed within a timely manner.
  5. Lead multi-disciplinary team meetings to develop individualized treatment plans that integrate medical care and psychosocial issues impacting patients.
  6. Provide supervision and direct job duties of the Behavioral Health Medical Assistant, to assist with case management needs, processing referrals and providing resources to patients as needed.
  7. Establish and maintain professional relationships with hospital employees as well as community agencies.
  8. Demonstrate strong communication skills in order to effectively coordinate services with collateral resources.
  9. Collaborate with medical providers during patient appointments to provide psychoeducation, crisis intervention and referrals for resources as needed.
  10. Attend collaborative behavioral health-based meetings within the hospital and community to enhance cohesive treatment.
  11. Responds to the needs of the department by performing other duties, as necessary. Participates in PCMH committees.
