Summary of Position:
The Registered Nurse for Concierge Medicine (RN) serves as the key point of contact within the health care system for patients to ensure timely access to a variety of health care programs and services. The RN acts as an advocate for the patient and facilitates care across the entire care continuum. The RN invests time to build meaningful relationships with patients and provides world class, comprehensive, customized care coordination. The RN is dedicated to a specific patient panel, though occasional cross-coverage for other RN's panels may be needed. The RN provides after hours call to perform nurse triage.

Qualifications
Education:
• Completion of an accredited nursing program
• Bachelor's degree in Nursing (BSN) preferred
Experience:
• Minimum of three (3) years as an RN in a hospital or outpatient clinic
• Experience working with patients with complex medical needs, such as those recovering from surgery or managing a chronic illness
• Experience in telephone or in person triage
Knowledge/Skills/Abilities:
• Excellent listening, communication and interpersonal skills
• The ability to harness independence and autonomy, when necessary, as well as the ability to work seamlessly as part of a team
• Flexibility and willingness to occasionally work after hours or on weekends, if necessary
• Sound clinical and personal judgment and professional demeanor
• Strong critical thinking and problem-solving skills and an ability to make quick decisions and take action in emergencies
• Proven nurse with track record of providing extraordinary patient care and customer service
• Advanced computer literacy
Certifications/Licensure:
• California RN license required
• Nevada RN license required
• Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider) or ability to obtain prior to start date.
• Valid California or Nevada Driver's license (depending on state of employee's residence) or ability to obtain prior to start date
• Current vehicle insurance

Essential Functions
1. Provides consistently exceptional care at all times.
2. The Registered Nurse - Concierge Medicine is responsible for triaging and coordinating the care needs of assigned patient panel and develops comprehensive plans to manage care delivery across the patient care continuum.
3. Corresponds directly with concierge patients on a routine basis, fostering a relationship of trust and reliability. Acts as the single point of contact for patients and referral sources ensuring high levels of customer satisfaction. Intervenes immediately when a potential bottleneck in our system may occur.
4. Facilitates the patient's participation in all decisions concerning care and appropriately advocates on their behalf.
5. Works closely with teams of doctors and patient care coordinators to ensure best patient health and satisfaction outcomes.
6. Responds to telephone calls and MyChart messages in a timely manner.
7. Ensures physician orders to initiate treatments and scheduling are available and viable; judiciously intervenes/connects with physicians to rectify issues. May assist with entering orders for the physician to co-sign.
8. Escorts patients to and from various departments for tests and treatments; assesses need for adaptive equipment, wheelchairs, etc. to support safety. Remains with patients as requested and pro-actively orchestrates or takes additional customer service measures and actions as appropriate, to ensure a high-quality patient experience.
9. In addition to working with patients in the outpatient medical offices, the RN prepares for and executes all-inclusive visits to patients' homes and offices. Acts as a liaison between home health agencies and physicians to coordinate discharge plan for patient care and establish post-discharge plan of care.
10. Coordinates prescriptions and refills with pharmacies locally and out of the area.
11. Intervenes appropriately to reduce anxiety and enhance patients' ability to follow-through with appointments and prescribed care.
12. Involves appropriate leaders in service recovery as necessary.
13. Provides after-hours and weekend call as required for concierge patients.
14. Responds to the needs of the department by performing other duties, as necessary.
