We want everyone who works at CVS Health to feel empowered by the role they play in transforming our culture and accelerating our ability to innovate and deliver solutions to make health care more personal, convenient and affordable. *Willingness required to travel on site in Illinois monthly. * *POSITION SUMMARY* This leader will oversee the team of practice transformation advisors, and collaborate to tailor and implement the Quality and Practice Transformation strategies across the IL provider network, with a focus on alignment of practice operations and capabilities with Medicaid member needs, and to optimize health outcomes and practice earning opportunities in P4P. In alignment with the VBC team, this leader will focus on implementation of clinical best practices to improve performance in collaboration with primary care provider partners. The role drives and supports achievement of the quintuple aim through focused efforts on provider experience and population outcomes. Using innovation, integration, and data, links the quality management activities to business goals. Health Care Quality Management includes, but is not limited to, QM Programs/Infrastructure, behavioral health QM, credentialing, delegation and regulatory accreditation support, clinical and service measurement and improvement and quality of care investigations. Accountable for health care quality projects and initiatives through direction setting and leadership. Assimilates information to proactively develop quality activities aligned with company strategies and values. Proactively builds strong teams and business relationships, both internally and externally. Serves as a resource and subject matter expert (SME) on aspects of the quality program to develop and influence business strategies. Strong interpersonal, communication, and presentation skills at all executive levels. *Required Qualifications* -One of the following licenses is required: RN, Pharmacist, LCSW -3+ years leadership experience in a Medicaid or Medicare managed care setting -3+ years experience within the following areas: HEDIS, STARs, population health, provider practice improvement, and proficiency in data analytics *Preferred Qualifications* -Clinical and / or Quality Certification (ex: CPHQ, Lean Six Sigma) -Preferred to reside within 3 hour radius of Chicago.   *Education* Bachelor's degree in health related field; Masters preferred. *Pay Range* The typical pay range for this role is: $91,386.75 - $196,800.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.    In addition to your compensation, enjoy the rewards of an organization that puts our heart into caring for our colleagues and our communities.  The Company offers a full range of medical, dental, and vision benefits.  Eligible employees may enroll in the Company's 401(k) retirement savings plan, and an Employee Stock Purchase Plan is also available for eligible employees.  The Company provides a fully-paid term life insurance plan to eligible employees, and short-term and long term disability benefits.
