Job Description: The Care Manager role involves the employee providing support services for the clients with consideration of dignity and privacy. Support services may include companionship, concierge, medication assistance, personal care services, transportation or other services requested/needed by the client as defined in their service plan.

Responsibilities:

  • Complete visits/tasks as scheduled (may be multiple clients in multiple locations).*
  • Attend required orientation, scheduled in-services and staff meetings.
  • Comply with all agency policies and procedures, including documentation requirements.*
  • Maintain client confidentiality.*
  • Provide services as defined on service plan, which may include companionship, concierge,             medication assistance, personal care or transportation.*
  • Observe and report any safety hazards found in the client's home, changes in client condition         or complaints to the supervisor.* 
  • All other duties as assigned.

Qualifications:

  • Graduate of an accredited High School or equivalent preferred.
  • Ability read, write and follow directions.
  • At least 18 years of age and has demonstrated competency.
  • If under 18 years of age, is a high school graduate or is enrolled in a vocational educational           program, and has demonstrated competency to perform the tasks assigned by the supervisor.
  • Experience with caregiving agency preferred
  • Reliable transportation.
  • Work positively and favorably with clients, families, and staff.
  • Demonstrate compassion, responsibility, and cheerful attitude.
  • Not listed on the Nurse Aide Registry/Employee Misconduct Registry with a finding against the         aide relating to client abuse or neglect or misappropriation of client property.
