Job SummarySupervisor/Lead Caregiver role involves the employee working with community leaders in identifying activities to promote program, performing the actions necessary to admit clients (admissions, assessments, service plan), provide support and guidance to caregivers within the area assigned (interviews, training, competency, supervisor visits) and, when needed, providing support services for the clients with consideration of dignity and privacy.  

 Qualifications:

  • Graduate of an accredited High School or equivalent preferred.
  • Ability read, write and follow directions.
  • At least 18 years of age and has demonstrated competency.
  • Reliable transportation.
  • Work positively and favorably with clients, families, and staff.
  • Demonstrate compassion, responsibility, and cheerful attitude.
  • Not listed on the Nurse Aide Registry/Employee Misconduct Registry with a finding against the         aide relating to client abuse or neglect or misappropriation of client property.


 Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations. 

 

  1. Complete community visits/tasks as scheduled in assigned area (including participating in or hosting community activities, meeting potential clients and performing admissions, assessments, service plan, interviews, training caregivers, completing competencies and supervisor visits).* 

  1. Attend required orientation, scheduled in-services and staff meetings. 

  1. Comply with all agency policies and procedures, including documentation requirements.* 

  1. Maintain client confidentiality.* 

  1. If needed, provide services as defined on service plan, which may include companionship, concierge, medication assistance, personal care or transportation.* 

  1. Observe and report any safety hazards found in the client's home, changes in client condition or complaints to the supervisor.*
