The Ambulatory Pharmacy Liaison provides for improved service levels for Hackensack Meridian Health (HMH) patients by seeking to supply medications prescribed by the patient's physician for discharge in a timely manner so that the patient can be discharged efficiently from an acute care HMH facility and transition directly to home. Assists with obtaining information necessary for prescription processing, helps prioritize workflow and facilitates delivery of completed prescriptions back to the patient prior to discharge. Ensures patient medication educational needs are met. Responsibilities include the job functions of an Ambulatory Pharmacy Technician as needed / required, but follows a unique job description.

The hours for this position are Monday - Friday 10:30am-7pm. No holidays, weekends or on-call.

Education, Knowledge, Skills and Abilities Required:

1. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
2. Ambulatory experience with knowledge of medications.

Education, Knowledge, Skills and Abilities Preferred:

1. One or more years of experience in a retail or hospital based ambulatory pharmacy.

Licenses and Certifications Required:

1. NJ Registration as Pharmacy Technician.

Licenses and Certifications Preferred:

1. Valid Certification from PTCB or equivalent.
2. Certification, acquired through a NJ Board of Pharmacy Approved testing agency.

Working Conditions:

Lifts minimum of 10 lbs., pushes and pulls minimum of 40 lbs. and stands minimum of 4 hours a day.
