Valley Health Systems, Inc., the parent company of Holyoke Medical Center (HMC), provides a broad range of physician services in offices conveniently located throughout Western Massachusetts.# We are seeking a#Physician Assistant / Nurse Practitioner to join our outstanding hospital employed Nephrology Practice, which has been newly formed with two supportive, esteemed physicians at HMC Kidney Associates, which is built on an already established practice in the Springfield/Holyoke area.## Duties include but are not limited to: Develop a strong presence to see new patient consult and follow-up To provide care for patients within the#practice by conducting and properly documenting history and physicals, consults and daily progress notes consistent with the Medical Center#s policies; communicating any new or worsening findings to attending physician involved in the surgical case in a timely manner. To prescribe medications and appropriate therapies with the consent of attending physicians and in adherence to applicable Federal and State Requirements, and in accordance to PA/NP Guidelines. To provide accurate and complete information regarding patient care for billing and compliance purposes.
