In addition, we are committed to promoting community health through education and special programs.#The Holyoke Visiting Nurses Association, Inc. is looking to hire a fulltime Home Care Admissions Nurse. Position Summary: The Home Care Admissions Nurse evaluates patients referred to home care services and determines the types of services to be provided. #Conducts interviews of the patient and family to determine the appropriateness for services.# Responsible for obtaining the initial and ongoing referral documentation from physicians, insurers and providers. Coordinates the development of home care plans specifying the frequency and duration of the services provided. #The Home Care Admissions nurse will complete admission, referral and insurance forms.# Requirements: Education:# Bachelors # Nursing (BSN) preferred. Licensure/Certification: Current MA Registered Nurse (RN) license CPR Certified Experience: One (1) to three (3) years home care experience Demonstrated computer / EMR skills
