What's a typical day as a Case Manager - Service Coordinator? You'll be:
• Providing service coordination to individuals residing in the community at large
• Maintaining current knowledge base of DOH policies, procedures, and regulations related to service coordination
• Helping individuals and families access appropriate programs, benefits, and other opportunities that improve their quality of life
