General Summary of Position
Delivers comprehensive and seamless services that bridge the gap between the clinical and self-management aspects of diabetes and cardiometabolic care. Is an integral part of the health care team provides collaborative, comprehensive and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. Supports and advocates for people affected by diabetes to optimize quality care. Promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.


Primary Duties and Responsibilities

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Provides quality diabetes self-management education and medical nutrition therapy based on assessed needs in individual and group classes which are in-person or via telehealth
  • Utilizes appropriate teaching techniques that are sensitive to the learning needs/preferences of the patient.
  • Follows the American Diabetes Association (ADA) Education Recognition Program requirements which are based on the National Standard of Diabetes Self-Management and Support Guidelines.
  • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient' assessed needs and goals and promotes successful self-management.
  • Documents all patient contacts/visits in the Electronic Health Record and outcomes data base according to the MedStar Diabetes Institute's guidelines in a timely manner.
  • Conducts MedStar Diabetes Boot Camp visit one and visit two, follows the patient until s/he is transferred to the Virtual Clinic, uses MedStar Diabetes Medication Algorithm to perform shared decision making and to collaborate with Boot Camp NPs in the development of a diabetes medication plan that is patient centric.
  • Collaborates, advocates and confers with nurse practitioners, primary care providers, endocrinologists and other diabetes stakeholders in developing patient centered diabetes plan.
  • Communicates/meets with the MDI leader and team regarding program updates.
  • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.
  • Cross covers with the inpatient diabetes education team as needed and based on availability.
  • Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings.
  • Provides non-diabetes medical nutrition therapy to referred patients if there if schedule allows.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
  • Participates in multi-disciplinary quality and service improvement teams.

    Minimum Qualifications
    Education

    • Bachelor's degree in Nutrition/Dietetics required

    Experience

    • 1-2 years Related experience as a registered dietitian. required and
    • Diabetes self-management education and support experience preferred

    Licenses and Certifications

    • REG DIET - Dietitian - Registered Valid license based on location of practice as a Registered Dietitian (must be registered by the Commission on Dietetic Registration). required
    • Certified Diabetes Care and Education Specialist - CDCES or BC-ADM credential preferred
    • If not already DCES or BC-ADM will sit for the certification examination within 1-1/2 Yrs

    Knowledge, Skills, and Abilities

    • Excellent verbal and written communication skills.
    • Basic computer skills including use of Microsoft Office.
    • Has working knowledge of diabetes technology.
    • Highly organized and strong attention to detail.
    • Strong problem solving and critical thinking skills.
    • Must be able to work with a diverse patient population and have exceptional customer service skills as defined in the SPIRIT values.
