Description
Responsible for the development and oversight of community and population health services as part of Care Transformation department for the Baltimore Region hospitals - MHH, MGSH, MUMH, and MFSMC. Position is key to the regional care transformation team and oversees external outreach divisions of each hospital, community health nursing, community benefit reporting, and community health needs assessments. Supports the strategy, design, implementation, and evaluation of the hospital's population health activities. Responsible for identifying opportunities for quality improvement for service delivery and outcomes, ensuring compliance with State and Federal regulations.
Education
- Master's degree in public health, social work, public administration, business required or
- related fields required or
- equivalent experience. required
- 5-7 years Progressively more responsible job-related experience. "thorough knowledge of maryland and federal health policyprograms impacting community health." Experience with or understanding of clinical considerations, and social determinants of health. Experience with computer systems required, including web-based applications and microsoft office applications which may include outlook, word, excel, powerpoint or access. required and
- Managerial experience; knowledge of cerner emr and crisp database preferred
- Valid RN license in the State of Maryland required
- Requires the ability to work in a rapidly changing environment while considering new and untested approaches.
- Requires excellent leadership, teamwork, and interpersonal communication skills; demonstrated ability to persuade, communicate clearly, and cooperate with other MedStar Health entities and MHH MGSH, MFSMC & MUMH clinical and non-clinical leadership to achieve overall organization objectives.
