Overview:

 

The coder is responsible for reviewing and analyzing documentation present in the medical record for inpatient, outpatient and/or professional services to assign diagnoses/procedure codes as described by the physician(s) of record. Abides by the Standards of Ethical Coding as set forth by the American Health Information Management Association and adheres to official coding guidelines.


Qualifications:

  • Completes Professional E/M Coding for inpatient, observation, outpatient,visits, & minor clinic procedures, and abstracting functions as assigned.
  • Demonstrates the ability to achieve accuracy and consistency in the selection of diagnoses and/or procedures. Identifies complications/co-morbid conditions.
  • Verifies point of origin codes, discharge status codes, and other registration/demographic data as appropriate.Coding Specialties include; Neurosurgery, Orthopedics, General Surgery, Surgical Trauma, ENT, Cardiology, Critical Care, Intensivists, and NeurologyConsistently meets or exceeds coding productivity standards per Mercy Coding Services Department productivity policy.Consistently meets or exceeds quality requirements per Mercy Coding Services Department policy.Outstanding 98-100% accuracyVery Effective 96-97% accuracyEffective 95% accuracyNeeds Improvement <95% accuracyEnsures compliance with CMS and other regulatory compliance guidelines, Joint Commission compliance, and Mercy guidelines.Queries physicians for clarification of documentation in a non-leading manner, as directed by coding guidelines and Mercy Coding Services Department policy. If an IP coder, participates in the documentation improvement process in conjunction with the Coding Supervisor as appropriate. Promotes and contributes to the collaboration between the local Documentation Review Specialist(s) and the Mercy Coding Services Department in a positive manner. Participates in all continuing education opportunities set forth by the Mercy Coding Services Department and acquires applicable continuing education hours to maintain certification(s). Promotes and contributes to the collaboration between the local HIM department and the Mercy Coding Services Department in a positive manner.Serves as a mentor to the Coding SpecialistPerforms other duties as assigned.
