Key Outcomes:

• Supports all clinical aspects of home-based population health programs and services including, but not limited to provider education and feedback, coding and clinical documentation, network development and management, APP oversight, readmission prevention, quality programs and outcome measurement.

• Works in conjunction with OHN leadership to understand and act upon opportunities to reduce unnecessary readmissions, hospitalizations, and ED visits. Will include presentations at operating reviews, and to payors, providers, and regional leadership teams as needed.

• Coordinates home based support programs with outside vendors and monitors for quality and performance in alignment with value-based performance goals and objectives.

• Provides clinical leadership and oversight to post-acute at home transition programs including, but not limited to home health certification and recertification programs, and APP home visits.

• Collaborates with Ochsner Connected Care in order to facilitate growth, feedback, and potentially design input of home based care programs.

• Supports the VP – Post-Acute and the VP – Care Management as physician leader in all clinical aspects of home-based population health programs and services.

• Supports the CMO of OHN and the Medical Director of OACN, as physician leader in all clinical aspects of population health home-based programs and services throughout OHN and OACN. Will include presentations at OHN/OACN committees, physician education for OACN and OHN participating providers inside and outside of Ochsner Health, and participation in payer meetings concerning home solutions.

• Supports the Chief Population Health Officer in initiatives and programs to reduce readmissions and unnecessary admissions and improve quality outcomes while reducing avoidable or unnecessary costs.

• Works collaboratively with the Medical Director of Post-Acute in programs to improve proper post discharge placement and care.

• Works collaboratively with the Medical Director of Clinical Documentation Excellence in programs to ensure proper documentation in the home setting.

Qualifications

Education/Experience:

• Medical degree with 5+ years of practice experience, Board certified in current specialty.

• Active and unrestricted license to practice medicine or surgery in Louisiana required.

• Medical director experience preferred but not required, with demonstrated knowledge of health care systems, including Medicare advantage and Medicare shared savings programs.

• Advanced degree (MBA, MMM), or progress towards such degree is preferred but not required.

Skills/Knowledge/Competencies:

• Knowledge and practical understanding of healthcare systems and healthcare quality and population health programs.

• Demonstrated ability to lead change (internally and externally) and produce measurable results within a specified time period.

• Demonstrated ability to lead, manage and collaborate with others.

• Good analytic skills with the ability to identify meaningful trends and targets for improvement.

• Excellent interpersonal skills and the demonstrated ability to establish rapport and working relationships with providers, payors, and internal staff.

• Strong communication and presentation skills; ability to tailor message to a variety of audiences.

• Successful utilization of process improvement tools and methods to achieve results.
