This position works Monday through Friday, with occasional Saturdays required.
JOB SUMMARY
- Acts as primary point of contact for patients after initial diagnosis / admission and until patient has sufficient knowledge and resources to self direct care. Has responsibility for monitoring, coordinating care, and educating wellness patients and families as they move through the system. It is the goal of the Nurse Navigator to assess the care service required by patients and make appropriate referrals and empower them through knowledge and emotional support to independently navigate services.
Qualifications
EDUCATION REQUIREMENTS
- 4 year / Bachelor's Degree in Nursing must be obtained within 1 year of starting position (Required)
- 2 - 3 years Nursing (Required)
- Required Certifications/Licensures: Registered Nurse (RN) with current Georgia license, Basic Life Support (BLS), Obtain Nurse Navigator Certification based on the criteria set by AONN ( must have 3 years of direct navigation experience).
