Job Summary
To educate the community about outreach programs through door-to-door community outreach, healthfairs, community events and contacts with community organizations. To assist patients with paperwork and provide staffing for community events.Accountabilities
- Educates community regarding outreach programs. 20%
- Assists screening participants with forms and paperwork. 20%
- Staffs exhibits at community health fairs and other community events. 20%
- Assists social worker with follow-up. 20%
- Reports outreach activities to the coordinator. 10%
- Conducts community presentations and facilitates community health fairs. 5%
- Attends monthly staff meetings and trainings. 5%

Minimum Requirements
Bachelor's Degree in Social Work, Public Health, or related field.
Two years experience in social work or community work
Required Certifications/Registrations/Licenses
Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.
Required Knowledge, Skills, Abilities
Basic computer skills
Knowledge of office equipment (fax/copier)
Proficient computer skills (word processing)
Effective at Data Conversion
