Job SummaryProvides clinical direction for all aspects of patient care in an ambulatory setting under the direction of the physician, Advanced practice provider, or physician resident. Demonstrates clinical competency utilizing evidence based practices/research in all areas of the nursing process: assessment, planning, implementation, evaluation and outcomes. Incorporates knowledge of safety protocols, cost containment and quality improvement throughout the nursing process. Adheres to all policies set forth from The Joint Commission, Board of Nursing, Board of Pharmacy, DHEC, CAP, COLA, and other regulatory agencies.
This position is eligible for a $10,000 sign-on bonus until October 1, 2021 for external candidates only.
Prisma Health Pulmonology
Our physicians are focused on diagnosing and treating lung-related diseases, providing specialized care for a wide variety of pulmonary conditions. We work to ensure that each patient receives the most efficient and thorough medical treatment possible.
By taking the time to review each patient carefully, we fit them with a clinician who best treats their needs. We provide inpatient care services at Prisma Health hospitals, including pulmonary and intensive care (ICU) consultations, and are skilled at various therapeutic and diagnostic procedures.
Collects data and completes nursing assessment utilizing appropriate area and/or age specific standards/policies and tools. Assesses and recognizes subtle or actual changes in patients' condition and patient response to intervention. Triages patient phone calls and manages clinical needs within scope of practice. Uses critical thinking skills to guide patient care. - 25%
Collaborates with Physician/ APP/ physician resident to develop plan of care for patient. Makes appropriate referrals based upon assessment (eg. skin, nutrition, social services). - 15%
Provides direct care to patients in a safe, timely, appropriate and cost effective manner; consistent with the established plan of care and identified patient priorities. Performs specific tasks based on organizational, departmental and/or policies/ procedures within the nurses' scope of practice; utilizing known resources and appropriate technology internal/external to the practice(per evidence based practice).
Delegates appropriate nursing tasks as appropriate to members of the healthcare team, ensures completion of assigned tasks. Monitors quality of care delivered.
Assumes responsibility for patient safety by maintaining a safe environment to include awareness of safety procedures, protocols and adherence to patient care standards (infection control, medication administration, etc.). - 25%
Notifies provider of changes in patient condition based on assessment/reassessment of patient response to interventions. Documents appropriately in medical record. - 15%
Identifies/assesses patient/family learning needs and teaches accordingly by:
- using language and terminology that the patient/family is familiar with
- providing examples to clarify ideas
- encouraging patient/family involvement in implementing and evaluating teaching plan
- utilizing appropriate resources (eg. written tools, computer aides, etc.)
Communicates the patient/family's need for education/teaching to others in the healthcare continuum, utilizing resources within the interdisciplinary healthcare team as necessary. - 10%
Evaluates own nursing practice in relation to professional practice standard guidelines, state statutes, policies, procedures, rules/regulations and current specific knowledge. Takes personal accountability for maintaining competency through education and professional activities to promote the healthcare of the community. Maintains professional licensure and completes required mandatory education in a timely manner. Reflects on identified areas of strength that offer opportunity for improvement.
Ensures that written documentation and oral communication focuses on pertinent information to support meeting patient/family care needs.
Serves as the patient's advocate and coordinator of care, working collaboratively with the physician(s) and other members of the healthcare team. - 10%
Education - Associate's degree in Nursing
1 year of nursing experience.
In Lieu Of The Above Minimum Requirements
In lieu of 1 year of registered nurse experience, will accept successful completion of the Prisma Health Nurse Residency Program.
