RVCC and HMC are#looking to hire a Licensed Clinical Social Worker (LICSW or LCSW) to work in their outpatient and oncology departments! # Overview of Job Responsibilities: The Licensed Clinical Social Worker will work#for River Valley Counseling Center and Holyoke Medical Center#Oncology providing trauma-informed assessment and treatment of clients# mental health needs. Evaluate and respond to the needs of individuals and/or their families and providing care based on the documented clinical assessment of the patient#s needs, which includes a culturally sensitive assessment of biophysical, psycho-social, environmental educational, and discharge planning factors. Including developing a clinical formulation that is based on assessment data and grounded in a clinically sound theory that directs treatment toward individualized, solution-focused interventions. Conduct clinical assessments, diagnose mental health disorders, and develop personalized treatment plans for clients. Providing ongoing therapy to individuals, families and/or couples, adjusting treatment plans as necessary and conducting ongoing assessments of progress. Utilize strength-based, trauma-informed, resilience-building communication strategies in interactions with clients and families. Exercise cultural humility and respect at all times. Coordinate any necessary referrals, outreach, advocacy, or case consultation as appropriate to meet client needs. Participate in client care and multidisciplinary team conferences when indicated. To provide comprehensive psychosocial care to patients with cancer and their families, including case management, needs assessment, and individual, group, and family counseling. Will support patient and family needs by providing referrals for economic, psychological, social, hospice, and home care. To meet with all new oncology patients and also set-up follow-up visits to patients. Some meetings could be via telephone or video call. To assist patients and/or their families with health related financial, social and emotional concerns and assist the patient with developing effective coping skills. To educate the patient and family members and other team members on medical social work interventions and treatment plans. # Job Requirements: Education Level -#Master#s of Social Work from a program accredited by the Council on Social Work Education (CSWE) Years of Experience -#Three years# experience as a social worker required Skills/Abilities - Bilingual (Spanish/English) ability desirable, though not required. Excellent verbal and written communication skills, interpersonal skills, crisis intervention skills, organizational skills, attention to detail, computer skills, good customer relations skills.#Must have knowledge of legal and ethical issues pertaining to the profession. Must be knowledgeable about disease, treatment, and the psycho-social implications for individual patients and their families. Facilitates a smooth transition for the patient through the continuum of care and appropriately access hospital, community and government resources, and assists the patient and families with end of life issues including hospice service coordination and advanced directive documents.
