Advanced Practice Provider (Nurse Practitioner or Physician Assistant), Liver Transplant Inpatient, Full-Time, Days and Nights, University of Chicago Medicine in Hyde Park, Chicago, IL

 

 

The Advanced Practice Provider (APP) is responsible for comprehensive direct patient care management. As an APP, you will manage a caseload of patients and exercise autonomous decision making in the assessment, diagnosis and initiation of delegated medical treatments. The scope of practice includes family support, discharge planning, staff education, and consultation. In this role you will provide direct support to patients throughout the inpatient stay for the abdominal, kidney, and liver transplant patients.

 

This individual will utilize advance practice skills obtained through the completion of a formal provider program. These skills include advanced levels of assessment, planning, intervention, and evaluation of care, as well as performing some invasive procedures. For this position, the APP will be responsible for functions related to the clinical care and management of solid organ transplant patients and others with end-stage organ disease. The APP is also responsible for planning, organizing, and directing patient care efforts in collaboration with medical and surgical physicians and other team members. Based on the specific work group, the individual's responsibilities may encompass the pre-transplant, peri-transplant/inpatient, post-transplant, or all phases of transplant care.

 

Nurse Practitioner (NP) Required Qualifications

  • Master's of Science in Nursing required
  • Current and/or eligibility for State of Illinois RN and APN licensure/registration required
  • Must be certified as a Nurse Practitioner by the American Nurse's Credentialing Center or American Academy of Nurse Practitioners
  • Must have or be able to qualify for Illinois and DEA prescribing numbers

OR

 

Physician Assistant (PA) Required Qualifications

  • Completion of an accredited (Physician Assistant) educational program required
  • Certification by the National Commission on the Certification of Physician Assistants (NCCPA) required
  • Current and/or eligibility for State of Illinois licensure/registration required
  • Certification in ACLS required
  • Must have or be able to qualify for Illinois and DEA prescribing numbers

 Position Details

  • Job Type/FTE: 1.0, Full-time
  • Shift: Days and Nights (Anticipate rotating from days to nights every 3 months)
  • Unit/Department: LIVER TRANSPLANT
  • CBA Code: Non-Union 

Preferred Qualifications

  • A demonstrated ability to effectively collaborate and communicate among interdisciplinary teams.
  • Must be highly proficient and organized with patient examinations and assessments, diagnostic interpretations, clinical interventions, and health care planning.
  • Must have effective problem-solving skills
  • At least 1-2 years of inpatient/outpatient APP experience preferred.
  • Experience in abdominal transplant and immunosuppression preferred but not required.
