Join University of Chicago Medicine, as a Clinical Pharmacist, Registry in the Meds2Beds Pharmacy. In this role as a Clinical Pharmacist, you will assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacists adeptly function in pharmacy operational areas, including but not limited to central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas. The Inpatient Pharmacy is known for providing exceptional assistance when your medical needs may be complex and requires special knowledge when collaborating with your medical provider and insurance company. Job Summary


Responsible and accountable for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient's symptoms, or arresting or slowing of a disease process, and improve a patient's quality of life in collaboration with a multi-disciplinary team. The pharmacist also assumes the responsibility for pharmacy technicians that work directly under his/her supervision by ensuring the proper preparation of drug products and managing the workflow.

 

Essential Job Functions

 

  • Maintains a standard of performance that is in compliance with JCAHO, ASHP practice standards/guidelines, federal/state licensing acts, and UCH and Pharmaceutical Services Policies and Procedures
  • Interprets, evaluates, and accurately dispenses all drug orders in the patient's best interest as assessed by appropriateness of drug selection, contraindications, dose, side effects, drug interactions, food interactions, duration of therapy, route of administration, and cost
  • Provides therapeutic monitoring of drug therapy including the monitoring of serum concentrations (i.e. Aminoglycosides, phenytoin, digoxin, warfarin, etc.) and the adjustment of drug dosages based on a patient's age, size, renal/hepatic function, severity of illness, and other patient specific characteristics
  • Identifies, resolves and prevents potential and actual drug-related problems. Recommends rational therapeutic alternatives as needed to prevent future problems
  • Participates in drug preparation and provides drug information in collaboration with the institution's Dr. Cart Team during cardiac resuscitation
  • Accurately checks medication orders that have been entered into the pharmacy computer system by the pharmacy technician: insuring that patient allergies, drug duplication, and drug interactions do not exist which contraindicate therapy and that the drug dose is correct. Enter orders directly into computer system if necessary
  • Coordinates with the technician the preparation and labeling of all first and missing doses (IV, PO, etc.)
  • Checks all medications prior to dispensing for accurate labeling, preparation, and storage. Coordinates all clinical and dispensing workflow that occurs from the satellite pharmacy
  • Supervises the quality/quantity of work and work performance of the pharmacy technician
  • Coordinates pharmacist and technician coverage of the area of responsibility during meal breaks or in-services. Assist other staff members, in other areas, in times of high workload
  • Supervises pharmacy technicians working under their direction to provide optimal pharmaceutical care and services
  • Participates in the orientation, education, and mentoring of new staff (i.e. pharmacists, nurses, pharmacy technicians, etc.) as needed
  • Participates in the preparation of articles for publication in the “Topics in Drug Therapy” or other UCH newsletters as requested
  • Performs other duties as requested by senior management

 Required Qualifications 

  • Graduate of an ACPE accredited College of Pharmacy
  • Doctor of Pharmacy or other advanced degree highly preferred
  • Eligible for licensure as a pharmacist in the state of Illinois (complete licensure process within 60 days of employment).
  • Completion of an ASHP accredited PGY-1 Pharmacy Residency required 

Preferred Qualifications 

  • Advanced Degree, Masters of Science, MBA, or Doctor of Pharmacy highly preferred.
  • Completion of an ASHP accredited residency PGY-2 highly preferred.
  • Clinical pharmacy experience in ICU highly preferred.
  • Extensive clinical experience and development may be considered in lieu of formal training

Position Details

In this role, we anticipate that you will generally work approximately around 24 hours per week.  We anticipate that you will generally be scheduled to work the rotating shift, starting from 7am to 11am, and will not be scheduled for on-call shifts.  We anticipate you will be scheduled to work 100% at the Hyde Park campus.  The days of the week that you are scheduled will vary, but you will work every other Sunday. 

 

Must comply with UCMC's COVID-19 Vaccination requirement as a condition of employment. If you have already received the vaccination, you must provide proof as part of the pre-employment process. This is in addition to your compliance with the Flu Vaccination requirement as well. Medical and religious exemptions will be considered consistent with applicable law. Lastly, a pre-employment physical, drug screening, and background check are also required for all employees prior to hire.
