Join a world-class academic healthcare system, UChicago Medicine, as a Patient Care Coordinator in the OB/GYN Department at our Hyde Park location as well as travel to our offsite  clinics as neededIn this role you will partner with the team to provide patients the highest quality of care at our offsite locations.

 

Under direct supervision responsible for guiding patients through the health care system and ensuring those patients and their caregivers are able to access culturally and linguistically appropriate services in a timely and cost effective manner. This position is responsible for coordinating appointments for new patients with multiple providers and ancillary appointments before and after their first visit. 

 

Essential Job Functions:

  • After initial appointment is scheduled serves as a point of contact with patients and is responsible to schedule appointments, including surgery, and coordinate referrals for testing, imaging, procedures, social work, physical therapy, occupational therapy, labs, and other care and support services at the hospital or in the community; contacts patients via phone, email, MyChart, and/or letter to schedule, confirm, and send out reminders regarding appointments.
  • Follows up with patients at regular intervals as defined to ensure that patients arrive to scheduled appointments on time and prepared. Fields patient and caregiver calls and questions related to appointments, ancillary services, or other matters related to coordination of care. Supports triage of In-Basket messages, and communicates effectively with hospital staff, clinicians and physicians involved in the care of the patient.
  • Obtains prior authorizations, medical records, slides, specimens, faxes, and other items necessary for coordination of care; follows up with outside facilities/labs and other ancillary services for testing (including imaging) results and medical records and shares results with appropriate services as needed, including submitting results to Medical Records and patient's community physician(s).
  • Initiates and prepares FMLA/Disability paperwork with patient, completes appropriate forms as directed by clinic staff, including Nurse Associates and Physicians; Documents in the electronic medical record, including information provided to patients and their caregivers about appointments and referrals; Creates EPIC notes for supporting documentation
  • Performs other related duties as assigned.
  • Builds relationships with hospital staff, clinicians and physicians involved in the care of the oncology patients and with members of the community with whom UCM has strategic partnerships with for referral services.

Required Qualifications: 

  • High School diploma required
  • Minimum of two years' experience working in a high-volume healthcare environment
  • Excellent telephone and personal communication skills are required as is proficiency with computer systems and programs
  • Ability to multi-task and prioritize is crucial to success in this position
  • Compassionate listening skills are required for patient and family distress related to new diagnosis
  • Ideal candidate has strong interest and desire for extensive patient and family interaction
  • Requires experience with and high commitment to customer service
  • Uses discretion and good judgment, and maintains patient confidentiality
  • Must be able to independently and professionally interact with clinicians and personnel, internal and external to UCM

 

Preferred Qualifications:

  • Bachelors preferred

In this role, we anticipate that you will generally work approximately 40 hours per week. We anticipate that you will generally be scheduled to work the day shift, and will not be scheduled for on-call shifts. We anticipate you will be scheduled to work 80% at the Hyde Park Clinic and 20% at our offsite locations. The days of the week that you are scheduled will be Monday through Friday.

 

Must comply with UCMC's COVID-19 Vaccination requirement as a condition of employment. If you have already received the vaccination, you must provide proof as part of the pre-employment process. This is in addition to your compliance with the Flu Vaccination requirement as well. Medical and religious exemptions will be considered consistent with applicable law. Lastly, a pre-employment physical, drug screening, and background check are also required for all employees prior to hire.
