Job Description
The Social Worker is responsible to facilitate care through resource coordination. This position helps patients achieve access to care and appropriate utilization of resources, balanced with the patients right to self-determination. The individual in this position has overall responsibility for and to assess the patient for discharge planning and transition needs, conduct complex psycho-social assessment and interventions to promote safe discharge preventing avoidable readmissions.
This position integrates national standards for case management scope of services including:
-Promoting appropriate length of stay while assuring care is the right place and right time.
-Compliance with state and federal regulatory requirements, TJC accreditation standards and Tenet policy.
-Education provided to physicians, patients, families, and caregivers
