Summary of Position:

Assists licensed staff in the provision of basic nursing and therapy care in Skilled Nursing, using the application of basic skills, restorative care, utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served; Adult through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team in meeting resident care goals.  The Certified Nursing Assistant SNF is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee

 

Qualifications

Education:

  • Completion of an accredited Nursing Assistant program

 

Experience:

  • Previous long term care experience preferred

     

    Knowledge/Skills/Abilities:

  • Experience or specialty training required in vital signs, bed making and resident transfer techniques
  • Sufficient computer skills as are required to complete an online application and the pre-employment/annual Net Learning requirements
  • In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred

 

Certifications/Licensure:

  • Current CA Nursing Assistant Certification or ability to obtain prior to start date
  • Current CPR/BLS certification by the American Heart Association (BLS Provider), American Red Cross (Professional Rescuer) or ability to obtain prior to start date

    Essential Functions

  1. Provide consistently exceptional care at all times.
  2. As directed by licensed nursing & therapy staff, provides care in a manner that is safe and sensitive to the rights of each resident.
  3. Performs hourly rounding consistently on every shift
  4. As directed, assists the care team by collecting resident care data at designated and appropriate intervals on all assigned residents. Reports finding to licensed staff.
  5. Recognizes and accurately reports relevant resident information to licensed staff. 
    • Reports on all significant changes in resident condition or motivational level.
    • Notifies speech therapy & occupational therapy of feeding problems, referrals and reassessment needs.
    • Notifies PT of any mobility problems, referral or reassessment needs.
  6. Utilizes time, equipment, and supplies in an efficient & safe manner. Completes all assigned duties during designated shift. Prioritizes resident care activities based on care needs.
  7. Provides personal care assistance to residents:
    • Personal hygiene – bathing, bathroom/bedpan, NOCcare
    • Positioning
    • Self-care activities - dressing
    • Ambulation & transfers
    • Eating
    • Transport
  8. Under direction of the therapist, performs routine physical therapy treatments
    • Range of Motion
    • Strengthening Exercises
    • Ensures placement of restorative devices
  9. Performs basic procedures & care to including:
    • Application of simple dressings
    • Specimen collection
    • Reinforces team management of bowel/bladder program
    • Assists residents with nutritional intake. 
  10. Communicates appropriate information regarding resident safety and infection control to RN or nurses manager.
  11. Ensures that all aspects of resident care are documented. Documentation is complete, accurate and timely.
  12. Supplies and Equipment
    • Operates equipment and ensures proper functioning of equipment
    • Follows procedure when equipment malfunctions.
    • Ensures use of supplies are accurately documented
    • Maintains accurate and complete documentation supply charges
  13. The staff member is able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the residents served.
  14. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards
  15. Adheres to all hospital and departmental policies regarding attendance and punctuality.
  16. Responds to the needs of the department by performing other duties, as necessary.
