Summary of Position:

Provides professional nursing care to the Skilled Nursing Residents, utilizes restorative nursing measures focusing on residents self care deficits, utilizing established policies and procedures, ensuring safe and effective care. Services are provided based on the ages of the population served; Adult through geriatric - in collaboration with the multidisciplinary care team.

Assesses, plans, implements and evaluates care in collaboration with the multidisciplinary care team. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all residents served in Skilled Nursing. The SNF Nurse is under the supervision and reports to the Director of Skilled Nursing

Qualifications

Education:

  • Completion of an accredited Nursing program

Experience:

  • One year of previous acute care experience preferred

    Knowledge/Skills/Abilities:
  • Sufficient computer skills as are required to complete an online application and the pre-employment/annual Net Learning requirements
  • In compliance with patient safety standards, must be able to effectively communicate in English; Bilingual abilities preferred

Certifications/Licensure:

  • Current California RN license or ability to obtain prior to start date
  • Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider)

    Essential Functions
  1. Provides consistently exceptional care at all times.
  2. Provides resident care, evaluates outcomes, consults with others as required and adjusts care plan as necessary to ensure optimal care.
  3. Performs systems assessment at designated and appropriate intervals on all assigned residents, recognizing disturbed physiology and abnormal parameters.
  4. Assesses pain levels on all residents;
    1. Upon initial assessment
    2. Reassesses according to departmental standards
    3. Evaluates location, severity, intensity and quality of pain.
    4. Uses the hospital approved pain scale for measurement
    5. Reevaluates pain level in response to intervention/ treatment
  5. Assesses psychosocial needs of resident, expressing interest in their progress and is sensitive to resident comfort and privacy.
  6. Based on assessment findings, documents IPN's as needed, completing alert charting as assigned and behavior charting
  7. Medication administration is safe, accurate and timely
    1. Follows all safety precautions related to resident ID, order confirmation, calculations and double-checking dose accuracy if applicable.
    2. Follows procedure when removing meds from floor stock (Narcotics, night lockers E-Kits, etc)
    3. Signs and ensures all narcotic wastage is witnessed.
    4. Documents all medication intervention to include rationale for PRN medications.
    5. Reports all Adverse Drug Events (ADE) according to policy.
ADE=ADR (Adverse Drug Reaction) & Medication Errors
  1. Monitors & evaluates outcomes of care. Reports any untoward change in resident condition to appropriate individual. Adjusts care to ensure optimal outcomes.
  2. Ensures the skin integrity of all residents is maintained through the provision of safe, effective prophylaxis treatment, ongoing assessment, staging and measurements according to hospital protocol.
  3. Recognizes unusual conditions outside of his/her expertise and seeks guidance of coworkers or supervisors.
  4. Communicates effectively with team members
    1. Supports team members offering assistance and guidance.
  5. Effectively coordinates scheduled daily care activities. Is self-motivated. Able to safely and effectively set priorities.
  6. Assists physicians with examinations, procedures and other processes related to direct resident care.
  7. Keeps resident/family informed of progress, procedures, and disposition. Focuses on customer service needs.
  8. The residents learning needs, abilities, preferences and readiness to learn is assessed. Effectively educates residents, families
    1. Medication use and safety
    2. Nutrition interventions
    3. Safe use of medical equipment
    4. Management of pain
    5. Rehabilitation techniques
    6. Use of other resources
  9. Monitors and ensures accuracy of documentation in resident medical record. All aspects of resident care are accurately and completely documented.
  10. Operates equipment and performs job related duties in a safe manner. Insures proper functioning of equipment and follows procedure when equipment malfunctions.
  11. Orients, instructs and trains assigned personnel
  12. The staff member is able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the residents served. The individual demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to identify each resident's age-specific needs.
  13. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards.
  14. Responds to the needs of the department by performing other duties, as necessary.
